Getting a letter or reference/recommendation
Please pull up this form for recommendation/reference or any other letter and print it. You may bring the printed, signed copy to the administrative assistant at the front desk and ask to have the letter dated when received and put in the appropriate person’s mailbox.
SPECIAL NOTE: IF YOU ARE OUT OF TOWN OR UNABLE TO HAND DELIVER THIS FORM, YOU MAY PRINT IT, FILL IT OUT, AND THEN MAIL IT TO: NURSING PROGRAM, HEALTH-CAREERS II; UNM-GALLUP BRANCH; 200 COLLEGE DRIVE, GALLUP, NM 87301.
- Please allow 14 days (2 weeks) for all your requests for letters.
- No letters or recommendations of any kind can be given with a phone call or verbal request only; this would violate your privacy rights as a student, i.e., the law does not permit anyone to do this.
- Letters will be provided only if students have given ALL required information on the appropriate form.
- Please ALWAYS include a contact telephone number and a regularly-read email address where you can be reached in case we need further clarification. Faculty, staff and/or the department chair will try to call one time with questions. With so many students and so many requests, it is impossible to chase down students. Again, if possible leave an email address that you refer to DAILY. This may be the easiest way for the letter-writer to get a hold of you. Thank you and best wishes on your endeavors!
